1. Definition
The
notion of a questionnaire is a technique of collecting data from a number of
people or respondents through a set of questions to be answered. By providing a
list of these questions, the answers obtained are then collected as data.
Later, the data is processed and concluded to become research results. Basically,
the purpose and benefits of the questionnaire are to obtain some data or
information that is relevant to the research topic. Generally, this method is
more widely used in quantitative research to describe the relationship between
variables.

questionnaire
The questionnaire is an open
questionnaire with a description question type. In this case, the respondent
needs to provide an answer in the form of an opinion or an explanation in the
blank column provided. Open questionnaires are usually used to obtain in-depth
information.
Ø Closed
questionnaire
Meanwhile, in a closed questionnaire,
the questions given are in the form of multiple choices or checklists. That
way, respondents can only answer questions with the available choices. Closed
questionnaire results are relatively easy to process and do not take much time.
Ø The
mixed questionnaire
Mixed questionnaire is a combination of
open and closed questionnaire types. Besides being useful for obtaining
in-depth information, this method also simultaneously provides a number of data
in the form of numbers.

(a) This
answer is not necessarily 100% valid because it is possible that the respondent
did not answer honestly.
(b) Respondents
tend to answer carelessly if the question is not understood.
(c) Respondents
are often less thorough so that they miss some questions.
(d) Can
only be done on respondents with a certain age range and education.
(e) The
questionnaire method will be difficult for those who are sick, cannot read, or
are old.

RESULT QUESTIONNAIRE
This
report is the result of my survey of the library in my school. This study
allows us to find out the opinions of students on school facilities, namely the
library. By way of voicing a group (23) of my classmates and different
classmates. When I asked whether they often read books in the school library
(15) the informant said yes, then was the school library comfortable for
reading, (12) the informant said yes. Then Usually, what do you read in the
school library 37% of them read more novels. The reason that made it
comfortable to be in the school library, the informants said because the room
was air-conditioned (15) informants and the rest answered because it was
comfortable and other positive responses. Finally, your opinion about the
school library 100% of the informants gave a positive and good opinion.
The
importance of the school library is to encourage the achievement of school
goals, namely education and teaching as outlined in the school curriculum. To
achieve a goal, the school library performs its function as an education
center, information center, and recreation center. As well as enriching the
learning experience other than in the classroom. Fostering the habit of
self-study and lifelong learning. Accelerate the mastery of the subject matter
delivered by the teacher. Help teachers obtain and arrange subject matter.The
reality in schools reveals that the library is still not functioning and has
not been utilized optimally by teachers and students. Teachers still tend to be
conventional in teaching. Such a learning system is less stimulating and does
not encourage students to read books provided by the school library, even
though the books are closely related to certain subjects or subjects. This
proves that there is a lack of harmonious communication between libraries,
teachers and students.
The
point is that the school library is able to function as a source of learning is
a must if there is a desire to improve the quality of students, teachers,
schools, even the quality of the nation. For this reason, it is to school
principals, heads of district/city education offices, regents/mayors,
governors, even the ministries of culture, primary and secondary education to
pay serious attention to the existence of school libraries as the heart of
education and one of the spearheads of progress. civilization of a nation so
that in the future our nation will no longer be looked down upon by other
nations in the world, but instead become a nation that is advanced, creative,
independent, highly competitive and most importantly a nation with dignity.
